Clinical Analysis of Intravenous Hyperalimentation



Abstract
A clinical review was made on 39 cases of surgical patients who were nourished with intravenous hyperalimentation in order to obtain the positive nitrogen balance, during last 3 years from January 1981 to December 1983 in the Seoul Red Cross hospital. Conclusion were as follows;
1) In all patients, the feeding catheter was introduced through the subclavian vein into the Sup. Cava.
2) Indications were 25 cases of E-C fistula (64.1%), 2 cases of hepatic failure, I case of cancer itself and 11 cases of malnutrition.
3) After hyperalimentation, total lymphocytic count was increased slightly in 23 cases (59%), markedly increased in 10 cases (25.6%) and slightly decreased in 6 patients.
4) Serum albumin level was increased in 8 cases, decreased in 16 cases and showed no changed in 15 patients.
5) Serum globulin level was increased in 34 patients (87. 2%).
6) Change of body weight after hyperalimentation was follows; slightly decreased in 7 cases (18%), no interval change in 16 cases and increased in 16 cases (82%).
7) During hyperalimentation, fasting sugar level was increased in 29 cases (74.4%) and administration of regular insulin was necessary in 4 patients (10.2%).
8) In the high output fistula (over 500m1), fistula was closed in 11 cases (62.1%) and mortality was 7 cases. All of the low output fistula was closed spontaneously. Overall mortality rate of fistula was 28%.
9) Complications were 2 cases of catheter infection, 1 case of pneumothorax, 1 case of air embolism and 1 case of hyperglycemic coma.
